im curently using media roadrunner and have installed the speedguide tweaks and they have helped , my q is if i were to try the cablenut program should i run the default windows patch first to set windows back to original reg or can i put it on top of sg ? thanks

OK, I haven't looked very hard, but does cablenut have a list of *all* his registry changes that I could look at?

I have been dubious to download and run an *.exe file -- eventhough I realize enough people here have tried it.

Nothing to fear with CableNuts progie. First, look at the settings you have now, write them down if you want. Then just use CB's settings. If they don't do anything for you delete them, this is built into the program, and then use CB's program to enter your old settings.
